[FONT="]Green urine arises when blue compounds are excreted by the[/FONT]
[FONT="]kidneys and mix with normal yellow urochrome pigments. A[/FONT]
[FONT="]number of agents have been known to cause green urine (for[/FONT]
[FONT="]example, indomethacin, amitriptyline, triamterene, Listerine,[/FONT]
[FONT="]flutamide, and injectable formulations containing phenol[/FONT]
[FONT="]such as some preparations of promethazine).[/FONT][FONT="]26 [/FONT][FONT="]Propofol is[/FONT]
[FONT="]excreted in the urine mainly as the 1-glucuronide, 4-[/FONT]
[FONT="]glucuronide and 4-sulfate conjugates of 2,6 di-isopropylphenol.[/FONT]
[FONT="]These are phenol derivatives, which cause a pH[/FONT]
[FONT="]dependent green urine discolouration that is more prominent[/FONT]
[FONT="]in alkaline urine. There are abundant case reports describing[/FONT]
[FONT="]green urine in patients with the propofol infusion syndrome,[/FONT]
[FONT="]a potentially fatal disorder marked by severe metabolic[/FONT]
[FONT="]acidosis, rhabdomyolysis, renal and cardiac failure particularly[/FONT]
[FONT="]in association with prolonged high dose propofol[/FONT]
[FONT="]infusions, in patients with brain injury.[/FONT][FONT="]27 28 [/FONT][FONT="]Case reports[/FONT]
[FONT="]may also be identified that record green urine in the context[/FONT]
[FONT="]of low doses of propofol, administered for sedation in[/FONT]
[FONT="]intensive care units and even after single anaesthetic[/FONT]
[FONT="]induction doses, unassociated with adverse complications.[/FONT][FONT="]29 30[/FONT]
[FONT="]The addition of dyes to enteral formulas is a bedside[/FONT]
[FONT="]technique for detecting aspiration in mechanically ventilated[/FONT]
[FONT="]patients. Blue dyes known to cause green urine discolouration[/FONT]
[FONT="]with their excretion include food drug and cosmetic blue[/FONT]
[FONT="]number 1, indigo blue, indigo carmine and methylene blue.[/FONT][FONT="]31[/FONT]
[FONT="]Intravenous infusion of methylene blue has been suggested[/FONT]
[FONT="]in the treatment of refractory septic shock.[/FONT][FONT="]32 [/FONT][FONT="]Conversely, in[/FONT]
[FONT="]recent times, concern regarding systemic absorption and[/FONT]
[FONT="]toxicity of these agents has been raised, particularly in[/FONT]
[FONT="]critically ill patients. Food drug and cosmetic dye number 1[/FONT]
[FONT="]may inhibit mitochondrial oxidative phosphorylation resulting[/FONT]
[FONT="]in lactic acidosis and impaired organ function.[/FONT]
[FONT="]Recommendations to stop using these agents for this purpose[/FONT]
